When do I pay for my visit?
All deductibles or co-pays are due at the time of visit—no exception. We accept Visa, MasterCard, personal checks (no third-party or payroll), and cash.

What insurance companies do you accept?
Presently the major insurance companies in our network include: Aetna, Anthem, BlueCross & BlueShield, Encore, Golden Rule, IU HealthCare, Wishard, Advantage, M-Plan, J.F. Molloy, Morris & Associates, Nyhart, United HealthCare.  If you are not sure whether or not your provider covers visits to the IU Center for Sports Medicine, please call your provider at the "1-800 number" located on the back of your insurance card.

Do you accept Workman's Compensation?
No, we do not.

Do I need a physician referral?
Most likely yes. All ''HMO" plans requires a referral from your primary care physician and most providers require a referral notice as well. Please call the "1-800 number" located on the back of your insurance card if you are unsure.

When can I make an appointment?
The clinic locations and appointment times can be found at the "Clinic Locations" page on this website. For all new patients, please arrive 15 minutes early to complete your chart forms (download your patient self-history chart now, page 1 and page 2, and bring it with you).

Do I need to bring anything with me?
Yes, please bring your insurance card with you for every visit. You will also need to either bring or have your physician courier-over any x-rays, x-ray reports, or other office notes from previous doctors visits. Additionally, runners should bring your running shoes when seeing the physical therapist.
